Cleft lip and palate are facial birth deformities. A cleft lip is a gap or discontinuity in the upper lip while cleft palate is an opening in the roof of the mouth, which divides the mouth and nose. Clefts result from incomplete development of the lip or palate during the week when the face is being formed. 

Nasal voice, trouble with speech, difficulty in eating or swallowing, chronic ear infections, impaired hearing, and dental problems are accompanied with cleft lip and palate. Treatment involves surgery to repair the defect and therapies to improve any related conditions.

Cleft lip and palate surgery

A cleft lip and cleft palate are usually noticed at birth, and your doctor may start coordinating with the healthcare team immediately. Cleft lip repairs are typically performed between 2 to 6 months of age and the cleft palate repair surgery is done before 18 months. In some cases, follow-up surgeries are required (during toddlerhood up to adolescence) to improve the aesthetical appearance of the nose and lip.

Preparation before cleft lip and palate repair surgery

The healthcare team members work together to develop a treatment plan for your child. Prior to the child's surgery, a pre-operation assessment like blood and urine testing and a chest x-ray are done. You will be instructed not to feed anything to your child for at least 10 hours before the cleft repair surgery. Also, make sure that your child does not have any viral infections like flu, cough, cold or diarrhea.

How is the cleft lip and palate repair procedure done?

The child would receive general anesthesia to make him/her unconscious. For cleft lip repair surgery, incisions would be made on each side of the cleft to create flaps of skin, muscle and intraoral tissue. These flaps are drawn together and stitched to close the cleft and recreate typical lip and nose anatomy. 

For the cleft palate surgery, incisions are made on either side of the cleft.  Depending on the child’s condition, specialized flap techniques are used to close the separation, reposition the tissues and rebuild the roof of the mouth (hard and soft palate). Once the cleft lip or cleft palates are repaired, the incisions are closed.

The procedure usually takes 2-3 hours, and the child would be monitored for another few days. In total, a duration of 7 to 10 days would be needed to undergo this procedure in India.
